BETWEEN Nutrition by Beenish, REGISTERED NUTRITIONAL THERAPY PRACTITIONER

(RNTP) AND CLIENT

PART 1

The Nutritional Therapy Descriptor

Nutritional therapy is the application of nutrition and lifestyle medicine sciences in the promotion of health, peak performance and individual care. Registered Nutritional Therapy Practitioners assess and identify potential nutritional imbalances and understand how these may contribute to an individual’s symptoms and health concerns. This approach allows them to work with individuals to address nutritional imbalance and help support the body towards maintaining health.

Nutritional therapy is recognised as a complementary medicine and is relevant for individuals with chronic conditions, as well as those looking for support to enhance their health and wellbeing. Practitioners consider each individual to be unique and recommend personalised nutrition and lifestyle programmes rather than a 'one size fits all' approach.

Practitioners never recommend nutritional therapy as a replacement for medical advice and always refer any client with 'red flag' signs or symptoms to their medical professional. They frequently work alongside medical professionals and will communicate with other healthcare professionals involved in the client's care to explain any nutritional therapy programme that has been provided.

The Registered Nutritional Therapy Practitioner (RNTP) requests that the Client notes the following:

  • The degree of benefit obtainable from nutritional therapy may vary between clients with similar health problems and following a similar nutritional therapy programme.
  • Nutrition and lifestyle recommendations will be tailored to support health conditions and/or health concerns identified and agreed between both parties.
  • RNTPs are not permitted to diagnose, or claim to treat, medical conditions.
  • RNTPs’ recommendations are not a substitute for professional medical advice and/or treatment.
  • Your RNTP may recommend food supplements and/or functional testing as part of your nutritional therapy programme and may receive a commission on these products or services.
  • Standards of professional practice in nutritional therapy are governed by the Complementary and Natural Healthcare Council (CNHC) Code of Conduct, Ethics and Performance.
  • This document only covers the practice of nutritional therapy within this consultation, and your practitioner will make it clear if he or she intends to provide services that are outside the scope of nutritional therapy practice.

The Client understands and agrees to the following:

  • I am responsible for contacting my GP about any health concerns.
  • If I am receiving treatment from my GP, or any other medical provider, I should tell him/her about any nutritional recommendations provided by my RNTP. This is necessary because of any possible reaction between medication and the nutritional programme.
  • It is important that I tell my RNTP about any medical diagnosis, medication, herbal medicine or food supplements I am taking as this may affect the nutritional programme.
  • If I am unclear about the agreed nutritional therapy programme/food supplement doses/time period, I should contact my RNTP promptly for clarification.
  • I understand that the recommendations are personal to me and may not be appropriate for others.
  • I must contact my RNTP should I wish to continue any specified supplement programme for longer than the originally agreed period, to avoid any potential adverse reactions.
  • Recording consultations using any form of electronic media is not allowed without the written permission of both me and my RNTP.
  • It is my responsibility to ensure I have a working internet connection for online appointments. If I am unable to connect as scheduled, the appointment is considered a ‘no show’ and charged in full.

If I am unclear about the agreed nutritional therapy programme/food supplement doses/time period, I should contact my Nutritional Therapist promptly for clarification.

I must contact my Nutritional Therapist should I wish to continue any specified supplement

programme for longer than the original agreed period, to avoid any potential adverse

reactions.

Recording consultations using any form of electronic media is not permitted without the

written consent of both me and my Nutritional Therapist.

Confidentiality and Data Protection

 

The Registered Nutritional Therapy Practitioner will keep your personal information confidential and secure following the UK GDPR guidelines for the practice. The RNTP will not share your information with third parties without your consent. However, if the RNTP believes there is a risk of significant harm to yourself or another person, the RNTP may pass the information onto an appropriate authority using the legal basis of vital interest. A separate Privacy and Consent Notice is available for your review.

PART 2

Payment Terms and Cancellation Policy

 

Fees are agreed in advance of the appointment and are paid in full before the appointment for face to face and online appointments. Package fees are paid in full in advance of the first appointment or if agreed in interest free instalments. Payments can be made by bank transfer, credit or debit card.

Appointments cancelled with less than 48 hours’ notice and ‘no shows’ will be charged in full.

In the case of advance payments for services, you have the right to cancel this Agreement within 14 days of the purchase without penalty (‘Cooling-off Period’). If you wish to terminate the agreement within the 14-day Cooling-off Period, you must do so in writing to info@nutritionbybeenish.com. The RNTP will refund any monies paid prior to cancelling this Agreement. If the services have commenced during the 14-day Cooling-off Period, the RNTP will not refund any monies due to you for services not yet provided. The service not yet provided will moved to another date.

Payments can be made by card via the online payment system. A debit or credit card

number will be requested at the time of booking when booking online.

If your therapist creates the booking/package for you, an invoice will be sent to you that is required to be paid in advance.

Instalment payment plans are available for packages only.

Any sessions that are missed or cancelled with less than 24hrs notice will incur a 100%

cancellation charge. Non-attendance of an appointment (no-shows) will also be charged in full.

If you do need to cancel an appointment at short notice, please let us know at the earliest convenience so we can reschedule your appointment. This is only applicable to online appointments. You can do this by emailing us at info@nutritionbybeenish.com

It is your responsibility to have a working internet connection for consultations via Video-

link. Full consultation charges will apply for scheduled sessions that are unable to connect.

There are no additional charges for phone consultations to a UK landline or mobile

number.

A 15-minute wait time frame will be applied to all sessions. Once 15 minutes of your

appointment has elapsed, and you haven't arrived at your session, the session will be

ended and will be classed a 'no-show'.

If you no-show for your appointment, you will be sent another invite to book a new session which will be deducted from your package. Refunds and reschedule options will not be offered for no-shows.

In your first session we will discuss and agree the support you require from your Nutritional Therapy, working together we will review the progress and aims as we go along. At this time you are required to sign the bottom of the page to agree to our terms and conditions.

Please speak with us if there is anything you would like clarified.

We understand the above and agree that our professional relationship will be based on the content of this Agreement. We declare that all the information we share during this professional relationship is confidential and to the best of our knowledge is true and correct.

Part 3 Physical Goods

Returns / Refund:

We do not provide a return or refund policy on any products sold on the website, Including finger prick tests. Although in case of any issues arising, we always try to help to your satisfaction.

Ordering and Cancellation Policy:

In-case of Cancellation you have 24hours to let us know via email. If any product is ordered on your behalf from a third-party, no refund will be issued. A credit note may be issued for future orders on Nutrition by Beenish own products.

Payments must be received in full for order fulfilment.

We reserve the right to terminate our agreement with you if we are refused authority from your card issuer for payment. You agree to compensate us in full against all reasonable costs and expenses that we may incur in obtaining payments due from you that you have not made in accordance with your agreement with us.

Please note that we reserve the right to change prices without prior notice.

We accept payment by credit or debit card via the website. We do not accept cheques or cash on delivery unless agreed in advance by a member of staff. If for any reason you do not wish to pay with a bank card, please get in touch and we can make arrangements for internet banking.

We may cancel or reschedule your order at any time before we begin the services in the case of an event out of our control, causes include, but are not limited to: power failure, internet service provider failure, fire, explosion, flood, storms, earthquakes, or other natural disaster.

Third Party Right & Links:

Nothing in these Terms and Conditions shall confer any rights upon any third party. The agreement created by these Terms and Conditions is between you and Nutrition by Beenish.

This website may contain links to other sites. Unless expressly stated, these sites are not under the control of Nutrition by Beenish. We assume no responsibility for the content of such websites and disclaim liability for all forms of loss or damage arising out of the use of them. The inclusion of a link to another site on this website does not imply any endorsement of the sites themselves or of those in control of them.

DISCLAIMER OF LIABILITY

While positive health outcomes are generally experienced by those

embracing a healthier attitude, lifestyle, and diet, Nutrition by Beenish does not make promises or guarantees regarding immunity from future illnesses.

Any reference to drugs during consultations is solely for the purpose of

obtaining a comprehensive history of the client's medication, and it is not

within Nutrition by Beenish's scope to assess the appropriateness of such medications.

Decisions regarding changes in prescription or dosage are to be made in

consultation with the client's physician.

Beenish Tariq is a qualified and registered Nutritional Therapy Practitioner and does not hold credentials as a physician or psychologist. The services provided by Nutrition by Beenish do not encompass the diagnosis or treatment of specific illnesses or disorders. If, as a client, there is a suspicion of having an ailment or illness requiring medical

attention, it is imperative to promptly consult with a licensed physician. It is

crucial to note that only a licensed physician can prescribe medications.

By affixing my signature below, I acknowledge my understanding that Nutrition by Beenish functions as a Nutritional Therapist and is not a physician.

I acknowledge the importance of consulting a doctor if I suspect a medical

condition. Nutrition by Beenish / Beenish Tariq will not be held liable for any failure to diagnose, treat, or prevent illness.

Additionally, I confirm that I have provided Nutrition by Beenish with a comprehensive and accurate account of any existing medical conditions and medications in my possession.

Rather than focusing on the treatment of diseases, Nutrition by Beenish directs attention toward wellness and the prevention of illness through the application of non-toxic, natural nutritional therapies to achieve optimal health. As a certified clinical nutritionist working within functional medicine framework, Nutrition by Beenish primarily serves to educate and motivate clients to assume greater personal responsibility for their health by adopting a healthy attitude, lifestyle, and diet.
